General Requirements:
- Must be a Philippine Resident and a Filipino Citizen
- Aged must be 21-65 years of age
- Gross annual income must not be lower than 120,000.00
- Employed as a regular employee for at least one year
- Or must be self employed for at least two years
Additional Requirements:
- For employed applicant - COE, payslips for the last three months and COE indicating the hiring date.
- Government issued ID (with photo) and company ID
- Foe self- employed applicants - audited Financial Statements (latest) or Income tax Return (latest)
